#Fill in Issuer Badge Number with your badge number along with your initials.
 
#Hand Radio to Staff Member
4. Write Time or Date checked out (Date is used for radios issued permanently to a staff member or area)

5. Fill in Issuer Badge Number with your badge number along with your initials.
