The Radio Checkout procedure is as follows.
1. Check number written on radio.
2. Write Number on page under Radio Number.
3. Have Staff Member write badge number and initial. (Make sure to check the badge)
4. Write Time or Date checked out (Date is used for radios issued permanently to a staff member or area)
5. Fill in Issuer Badge Number with your badge number along with your initials.
6. Hand Radio to Staff Member
7. When radio is returned, write the Time or Date returned.
